An unannounced renewal study was conducted today. There are 11 children (33 points) with 2 adults. An additional assistant needs to be present. 1 adult and 3 children's files were reviewed. See violation notice for missing information. I child has no file available for review; however, a sibling is present for emergency numbers. A file for each child needs to be on site. Children were observed during diapering, feeding and enjoying free play. There are currently no children receiving medication at the home. Emergency evacuation drills, fridge temperature, labeled bottles and the menu are all in compliance. Due to case load adjustments, your next inspection will be with a different inspector. All inspectors have ID badges. LI was present today from 9:45 AM - 11:10 AM.

Description: Based upon a review of files, one child's file is incomplete. Evidence: Child 1 does not have a completed physical on file. Date of enrollment (DOE): 8/24/15.

Plan of Correction: I will ask the parent again for the physical.

Standard #: 22VAC40-111-60-A
Description: Based upon provider's statement, one child does not have a file available for review. (Child 2)

Plan of Correction: I will ask the parent to bring the paperwork.

Description: Based upon a review of files, one child's file is incomplete. Evidence: Child 3's file is missing documentation of a second emergency contact.

Plan of Correction: I will call the parent and ask her to provider the information.

Description: Based upon a review of files, children's files are incomplete. Evidence: Child 2 does not have an immunization record on file prior to the first day of attendance.

Plan of Correction: I will ask the parent to provider the shot record.

Description: Based upon a review of files, one child's file does not contain an acceptable immunization record. Evidence: Child 1's immunization record on file is not signed, stamped or electronically signed by the physician's office.

Plan of Correction: I will remind the parent again to bring in the signed record.

Description: Based upon direct observation a child was in a seat without the safety strap buckled.

Plan of Correction: The strap was buckled during the inspection.

Standard #: 22VAC40-111-570-A
Description: Based upon a review of files and direct observation, the allowable 16 points per adult is exceeded. Evidence: There are 11 children present, aged: 4 months (2), 6 months (1), 11months (1), 17 months (1), 23 months (2), 2 years (2), 3 years (2), totaling 33 points with 2 adults.

Plan of Correction: I will hire an assistant.
