Technical Assistance:
Discussed the assistant ( providers daughter) applying to have the license at this address, the licensee is thinking about retiring and his daughter would take over. She is waiting to get her diploma from Nepal, so she can apply.

Comments:
The provider and two assistants are present with 5 children in care. A total of 12 points today. The younger children are playing with one of the assistants and the older three are writing and learning about animals, planets and numbers. Lots of academics going on during the visit. The children are coloring and interacting with the caregivers. Food is brought from home by the children. One child ate her breakfast when she arrived. This visit was started at 10:05 and ended at 11:45. Please call me at 703-203-8659 if you have any questions regarding this visit.

Standard #: 22VAC40-111-830-B
Description: The provider failed to conduct the second shelter in place drill of the year. Evidence: Based on the review of the emergency drill records and conversation with the assistant, the provider only had one shelter in place drill last year. The drill was conducted in July of 2015.

Plan of Correction: We will remember to have two this year.

Standard #: 22VAC40-111-100-A
Description: The provider failed to get a physical completed by a physician for one child. Evidence: Based on the review of 4 children's records , one child, child A is missing a completed physical, a physical that has the front page filled out by the parents but nothing else is filled out is in the file.

Plan of Correction: Will ask the parent to bring a completed Physical.

Standard #: 22VAC40-111-60-B
Description: The provider failed to get some of the information needed in the children's records. Evidence: Based on the review of 4 children's records, one child, child B is missing employment information for both parents.

Plan of Correction: Will ask the parents for the information.

Standard #: 22VAC40-111-90-C
Description: The provider failed to get updated immunizations for one child in care. Evidence: Based on the review of 4 children's records, one child , child D does not have updates of the immunizations, the last immunizations given were in 2012.

Plan of Correction: Will ask for it.
