Comments:
An unannounced monitoring inspection was conducted today from 10:50 - 12:10. There were 5 children = 11 points with two caregivers. One of the children is the licensee's child. The children were observed participating in a variety of activities including reading books, doing puzzles, playing with toys on the floor, eating lunch and getting ready for nap. The exit interview was conducted with the licensee. The violation notice will be mailed for completion.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on a review of written records of emergency evacuation procedures, the licensee did not document one emergency evacuation. Evidence - The emergency evacuation record form was completely filled and did not include a record of an emergency evacuation being conducted in January 2016.

Plan of Correction: Due to printer malfunctions was unable to have the form ready. In future will keep hard copy a [sic] file to prevent this from happening again.

Standard #: 22VAC40-111-540-A
Description: Based on observation, the licensee did not fasten the safety strap on the blue chairs. Evidence - There were two children observed being placed in blue chairs without the safety straps fastened.

Plan of Correction: Children sitting on blue chairs no longer in need of seatbelts. However, I [sic] be sure to follow procedure in future.
