Technical Assistance:
Discussion was held on the topics of current enrollment and current staffing. The training hours will be rechecked during the next inspection to correspond with the end of the current training year. The MAT certification previously obtained has expired. The Provider stated that she has rescheduled a course in order to obtain an updated MAT certification. There is currently no prescription medication on site according to the Provider.

The following inspection took place between the hours of 10 am through 12 pm (noon). Today, I met with the Provider on site at her Family Day Home (FDH) located near Aldie, Virginia. At the start of this unannounced monitoring inspection there were a total of 10 children (23 points) with 2 caregivers. A total of 3 children?s files and 4 staff/household member?s files were reviewed. The children were engaged in the following: bathroom breaks, hand washing, freeplay, a game involving colors, lunch, and other organized activities. Description: Based on review and interview, 2 of 4 staff/household members' files reviewed indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening (TB) documentation, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence - 1. On the date of inspection (02/10/2016), the following TB documentation was observed on site at the Family Day Home (FDH): Member #3 - TB documentation was dated - 01/06/14. This corresponds to the same date recorded during the previous inspection that took place on 09/08/15. For Member #1 - TB documentation was dated - 12-31-18. This does not correspond to the date recorded during the previous inspection that took place on 09/08/15. During that inspection the date that was recorded at that time for the TB documentation of Member #1 was 12-31-13. It is unclear of the post date of 2018 on this documentation. 2. The Provider stated (during an interview conducted on the date of inspection) that she did not know why the documentation was dated as such and did not have any further TB documentation for Member #1.

Plan of Correction: New documentation will be obtained for both Members.
