Technical Assistance:
Discussed training, enrollment, hand washing and background checks with the provider.

Comments:
The provider and an assistant are present with 5 children during this monitoring visit, one is the providers own daughter. A total of 20 points today. Observed the children playing, reading books, having a morning snack and bottle. Lots of interaction between caregivers and children observed during the visit. The provider has lots of space and lots of toys for the children to play with. Children bring their own food from home. This visit was started at 9:40 and ended at 11:00. Violations:
Standard #: 22VAC40-111-60-B
Description: The provider failed to get proof of ID for one child in care. Evidence : Based on the review of 4 children's records, one child, child C is missing proof of ID.

Plan of Correction: I already contacted the parent and will have the information tomorrow, I think I saw it just forgot to record it.

Standard #: 22VAC40-111-130
Description: The provider failed to get a background check conducted for one caregiver. Evidence: Based on the review of 2 caregiver records, caregiver A is missing a CPS check.

Plan of Correction: I will send one in, I did not realize that she needed it.

Standard #: 22VAC40-111-170-A
Description: The provider failed to get a TB screening for one staff. Evidence: Based on the review of caregiver records the provider did not get TB screening results for one caregiver who started working at the end of November 2015.

Plan of Correction: I will make she gets a screening.

Standard #: 22VAC40-111-280
Description: The provider failed to keep cleaners inaccessible to the children. Evidence: Based on the observation during the visit, the bathroom sink cabinet in not locked and accessible, the cabinet contained disinfectant cleaning wipes. The providers make up was on the bathroom counter and a bottle of resolve and cleaning wipes were left by the couch in the area used for children.

Plan of Correction: I added the lock, removed the cleaners and removed the make up.

Standard #: 22VAC40-111-690-D
Description: The diaper changing pad has small tears in it.

Plan of Correction: Switch with a different pad during the visit.
