An unannounced monitoring inspection was conducted on 2/24/16 from 9:05am - 10:30am. At the time of entrance there were 10 children ages 6 months to four years old (28 points) in care with two caregivers. A tour of the home was conducted and children were observed playing inside with toys, doing arts and crafts projects, and eating. Records were reviewed for three children and three provider/assistant provider/household member. There is no medication at the family day home. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-111-60-B Description: Based on a review of three children's records, it was determined that the provider did not ensure they maintain and keep at the family day home a complete record for each child enrolled that contains all required information. Evidence: 1. The record for child #3, present during the inspection, did not contain documentation of parent notification for the provisions of the family day home's emergency preparedness and response plan. 2. The provider reviewed the record for child #3, and confirmed the record was not complete. Plan of Correction: Not available online. Contact Inspector for more information.
