Today an unannounced monitoring visit was conducted beginning at 12pm. There were 7 children (15 points) present today with the Provider. Children were observed finishing lunch, cleaning up, and napping. New children's records (including children who have returned after an extended breaks) were reviewed today in addition to adult records. There are no children with medications on-site at this time. No violations were observed in the physical plant. Space for children continues to be open and provide ample age and stage appropriate supplies and equipment for children. Standard #: 22VAC40-111-60-B
Description: Based on record review, five children's records (#1,7,8,9,10) are missing complete required documentation in their record. Evidence: Children #1,8,9,10 are missing documentation that parents have reviewed and updated emergency information annually. The last documented review of information is dated 4/28/13 for child #1, 6/8/14 for child #8, 10/20/13 for child #9 and 7/4/14 for child #10. Child #1 is also missing documentation of a second emergency contact other than parent or guardian. Child #7 is missing documentation of a physical examination and proof of identity.

Plan of Correction: I will have all parents review, update and date the review of emergency information today when they pick up their children. I will also ask parents to provide me with other missing documentation.

Description: Based on discussion and record review, no adult caregivers have a current CPR training certification. Evidence: CPR training for caregiver #1 expired 8/2015.

Plan of Correction: I will register to take a recertification as soon as possible and keep certifications current.

Description: Based on discussion and record review, no adult caregivers have current First Aid training certification. Evidence: First Aid training for caregiver #1 expired 8/2015.
