Comments:
A joint investigation was conducted on 01/21/2015 from 12:00 p.m. to 2:00 p.m. and 01/27/2016 from 11:00 a.m. to 1:00 p.m. with the local agency to investigate a complaint regarding care of children. Interviews were conducted, staff records, children's records and documentation were reviewed. The complaint regarding care of children was determined to be not valid. The violation cited is not complaint related.

Description: Based on review of one staff record it was determined that not all staff were given orientation training by the end of their first week of assuming job responsibilities. Evidence: Staff A with a start date of 12/07/2015 did not have documentation of receiving orientation training.

Plan of Correction: Staff will receive orientation training before the end of the first week of assuming job responsibilities moving forward.
