54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced renewal inspection was initiated today at 1:30pm. The physical plant, 3 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 9 children (plus the Provider's child), totaling 24 points, under the supervision of 2 caregivers. During the inspection, children were observed enjoying free play, being fed (infant), having a diaper changed, socializing with the other children and resting at nap time. Since there was no school today, there were 3 school age children in care today. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served spaghetti, salad, garlic bread, applesauce and milk. The Provider is currently certified in MAT (medication administration training), CPR and First Aid. Description: Based on file review, the Provider does not have documentation of evacuation drills having been completed in January or February 2016.

Description: Based on interview, the Provider does not have a menu for the current one-week period.

Description: Based on file review, 1 out of 4 children's files does not have the results of a physical within 30 days of attendance. Evidence 1. Child #4 began care on 10/27/15. This child does not have a physical on file.

Plan of Correction: The Provider will ask the parents for this.

Standard #: 22VAC40-111-690-D
Description: Based on observations, the diapering surface is absorbent and unable to be disinfected after each use. Evidence 1. The diaper mat had a cloth covering over it while a child was being changed.

Plan of Correction: The Provider took off the cloth covering. She will make sure to disinfect the surface after each use.

Description: Based on file review, 1 out of 3 caregiver/household members does not have a criminal history record check (CRC) before 30 days of working at the family day home ended. Evidence 1. Caregiver/household member #3 does not have a CRC on file.

Plan of Correction: The Provider will send this off to be completed by Monday.
