Consultation provided on the repeat and high risk violations.

Comments:
An unannounced monitoring inspection was conducted with the provider, who was present with four children for a total of 9 points. The children were finishing breakfast ( cereal and milk) at the beginning of the inspection. Four children's files were reviewed today. The provider does not administer medication nor have their been any documented injuries/accidents. Please submit your plan of correction (to include how you plan to fix the violation, what systems you plan to put into place to make sure the issues aren't ongoing and the date each violation will be addressed) to your Licensing Inspector by 3/4/16. Standard #: 22VAC40-111-830-A
Description: Based on a review of the emergency evacuation drills, it was determined that the provider has not conducted monthly drills. Evidence: There has been no emergency evacuation drill conducted in January 2016.

Standard #: 22VAC40-111-830-B
Description: Based on interview with the provider, it was determined that there have been no practiced shelter in place drills twice a year. Evidence: The provider has failed to do shelter in place drills for 2014, 2015 nor has she conducted any in 2016.

Standard #: 22VAC40-111-60-B
Description: Based on a review of four children's files, it was determined that one child did not have all of the emergency contact information. Evidence: Child B's two emergency contacts did not list the street number and name for either contact. Child B enrolled on 2/10/14.

Standard #: 22VAC40-111-210-A
Description: Based on an interview with the provider, it was determined that she has not obtained 16 training hours annually. Evidence: The provider has 11.5 of the 16 required training hours for 2014 and none of the 16 hours required for 2015.

Plan of Correction: An Intensive Plan of Correction (IPOC) will address this violation.

Standard #: 22VAC40-111-430-B
Description: Based on observation and interview with the provider, it was determined that a pressure mounted gate was used as protective barrier at the opening of the stairs. Evidence: The Licensing Inspector observed a pressure mounted gate being propped at the top of the stairs to prevent a 16 month old ( child A) from going down stairs. The gate was propped and the 16 month old on two occasions was able to go around the gate and started to go downstairs. These gates may give way under a child's weight and allow a child to fall.
