Comments:
An unannounced focused inspection was conducted with the provider from 1:50 p.m. to2:20 p.m. There were three children in care ranging in age from one year to two years old for a total of nine points. Standard #: 22VAC40-111-640-2
Description: Based on observation and interview with the provider, it was determined that the provider used a forbidden action by placing a child in a space that they could not freely exit themselves. Evidence: A two-year old child was strapped into an infant car seat as a means of behavioral guidance. The infant car seat was not being used for its intended purposes.

Plan of Correction: 1. A forbidden action will no longer be used. 2. The infant car seat will be used for its intended purpose
