Conducted an unannounced monitoring study with the provider and three children for a total of eight points. The ages of the children range from six months-old through three years-old. The children were napping during the inspection. Two household records and three children's records were reviewed. The provider is current in First Aid and CPR. Start time: 1:00pm End time: 3:00pm
Standard #: 22VAC40-111-100-A Description: Based on a review of two children's records, one record was incomplete. Evidence: Child A with an enrollment date of 2/16/16 was missing documentation of a physical examination and was due on or by 3/16/16. Plan of Correction: The provider will get the missing information from the parents.
Standard #: 22VAC40-111-90-A Description: Based on a review of two children's records, two records were incomplete. Evidence: Child A with an enrollment date of 2/16/16 and Child B with an enrollment date of 2/22/16 was missing documentation of the immunization records, which are due on the first day of enrollment. Plan of Correction: The provider will get the missing information from the parents.
