Comments:
An unannounced renewal inspection was conducted today, 3/17/16. The violation from the previous inspection has been corrected. A violation notice was completed regarding the violations found during today's inspection. Upon arrival to the family day home the children were busy playing with the available toys. Diapering, hand washing, lunch and nap was also observed. There are nine children in care totaling 20 points with two caregivers. A review of adult records and four children's records were reviewed for compliance. Should you have any questions please do not hesitate to call. Inspection began: 10:40 am and ended 2:00 pm.

Violations:
Standard #: 22VAC40-111-830-B
Description: Based on observation of the required records the provider failed to conduct shelter-in-place drills twice a year. Evidence: 1. There is only one shelter-in-place drill documented for 2015. (6/6/15)

Plan of Correction: The provider will ensure that two shelter-in-place drills are conducted and documented each year.

Standard #: 22VAC40-111-210-A
Description: Based on review of staff records during the renewal inspection it was found that the providers did not obtain the required amount of annual training hours. Evidence: 1. Staff # 1has a total of four (4) hours of annual training and staff #2 has a total of two (2) hours of annual training.

Plan of Correction: The provider and assistant will obtain the required 16 hours of annual training.

Standard #: 22VAC40-111-280
Description: Based on observation of the family day home during the renewal inspection, the provider failed to have all potentially poisonous materials inaccessible to the children. Evidence: 1. The back part of the basement has the refrigerator, washer and dryer, table that is used by the school age children and several shelves. 2. Located on those shelves are cleaners and paints. These items are accessible to the older children.

Plan of Correction: The provider will place these items in an area that is inaccessible to the children in care. This will be monitored to ensure they are always inaccessible to the children.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of family members records the provider failed to obtain repeat background checks within the required time frames. Evidence: 1. Staff #3 has a criminal history record check that expired 8/16/15 and a central registry check that expired 8/13/15. These background checks are to be repeated every three years.

Plan of Correction: The provider will have these repeat background checks completed immediately. The provider will monitor the files to ensure that all background checks are repeated within the three year time frame.
