An unannounced monitoring inspection was conducted today. There are 7 children present (15 points) with 2 adults, including 2 of the provider's own children. 2 children's files were reviewed. See violation notice for missing information. Children were observed washing hands, using the bathroom, and eating morning snack. Standard #: 22VAC40-111-50-A Description: Based upon a review of documentation, a daily attendance sheet was not kept on Monday 3/21/16. Plan of Correction: I will have the parents sign the children in as they arrive.
Standard #: 22VAC40-111-70-A Description: Based upon a review of files, one child's file did not contain written acknowledgment of receipt of policies and procedures. Evidence: Child 1's file does not contain a signed Information for Parents (model form). The form contains all required policies and procedures that are to be given to the parents. Plan of Correction: I will ask the parent to sign the form tonight.
Standard #: 22VAC40-111-540-A Description: Based upon direct observation, a safety strap was not fastened while child was seated at the feeding table. Plan of Correction: I will remember to fasten all seat belts.
