- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, March 3, 2016 from 9:50 am to 11:10am. The children were observed playing with toys and listening to music. Today, there were nine children, totaling 25 points, present with two caregivers. Lunch consisted of: fish sticks, string beans, fruit cocktail and milk Three children?s records, two caregivers? records and one household record were reviewed during the inspection. Last emergency drill: 2/23/2016 Last shelter-in-place drill: 12/28/2015 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-730-D Description: Based on observation on 3/3/2016, the provider did not ensure that medications were kept in a locked place. EVIDENCE: Nonprescription medication was observed in a cabinet in the kitchen. The cabinet is out of the reach of children. Medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children. Plan of Correction: Will get a lock box and put in a locked cabinet.
Standard #: 22VAC40-111-920 Description: Based on observation on 3/3/2016, the provider did not ensure that a menu for the week was posted or given to parents. EVIDENCE: The provider provides the children enrolled meals and snacks. A menu for the week was not posted. When meals or snacks are provided by the family day home the menu for the current one-week period shall be given to parents or posted or placed in an area accessible to parents. Plan of Correction: Will post the menu before lunch.
Standard #: 22VAC40-111-60-A Description: Based on a review of children's records on 03/03/2016, the provider did not ensure that one of the nine children in care had a record at the family day home. EVIDENCE: Child #3, who was in care on 03/03/2013 did not have a record at the family day home. The provider shall maintain an up-to-date record at the family day home for each enrolled child. Plan of Correction: Will remind parent to bring forms today, 3/3/2016.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records on 03/03/2016, the provider did not ensure that one of the two records reviewed contained the name, address and phone number for two emergency contacts. EVIDENCE: The record of Child# 2 did not contain documentation of the name, address and phone number for two emergency contacts. A child's record shall contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. Plan of Correction: Will have parent complete at pick up.
Standard #: 22VAC40-111-150-D-1 Description: Based on a review of caregiver records on 03/03/2016, the provider did not ensure that an assistant that is left alone with the children has a current certification in cardiopulmonary resuscitation (CPR). EVIDENCE: Caregiver #2 does not have a current CPR certifications. Caregiver #2 was alone for approximately 15 minutes with nine children in care on 03/03/2016. An assistant 18 years of age or older who is left alone with children in care shall have a current certification in cardiopulmonary resuscitation (CPR). Plan of Correction: Won't leave assistant alone until get certifications.
Standard #: 22VAC40-111-150-D-2 Description: Based on a review of caregiver records on 03/03/2016, the provider did not ensure that an assistant that is left alone with the children has a current certification in first aid. EVIDENCE: Caregiver #2 does not have a current first aid certification. Caregiver #2 was alone for approximately 15 minutes with nine children in care on 03/03/2016. An assistant 18 years of age or older who is left alone with children in care shall have a current certification in first aid. Plan of Correction: Won't leave assistant alone until get certifications.
Standard #: 22VAC40-111-520-A Description: Based on observation and caregiver interview on 03/03/2016, the provider did not ensure that a crib was provided for infants under 12 months of age. EVIDENCE: Child #1 is nine months old and Child #4 is six months old. There wasn't a crib available for the child to sleep in. According to Caregiver #1, Child #3 rests in a mesh side baby rocker that was provided by the child's parent. Cribs shall be provided for children from birth through 12 months of age. Plan of Correction: Had ordered a crib, but had to return because of missing parts. Will order again.
Standard #: 22VAC40-111-570-A Description: Based on observation on 03/03/2016, the provider did not ensure that a caregiver did not exceed 16 points. EVIDENCE: At the inspector's arrival there were seven children in care totaling 19 points with one caregiver,Caregiver #2. Caregiver #2 stated that Caregiver #1 was out of the home and would return in a few minutes. After ten minutes Caregiver #1 returned to the home. Caregiver #1 stated that when they left the home that morning Caregiver #2 was within the required points. Plan of Correction: Will make sure not to leave Caregiver #2 over points.
