An unannounced renewal inspection was conducted with provider and two assistants. Two children were getting picked up, leaving four children present totalling 11 points. Many children are absent due to Spring Break in Stafford County schools. The children were having free play in the day care area and watching videos on the iPad. Two children's files were reviewed - please see Supplemental Summary for missing items. The children had an art project earlier - painting pigs with chocolate pudding. Provider does not administer medication - Medication Administration Training is required to administer medicine. Standard #: 22VAC40-111-100-A Description: Based on review of two children's files, the provider failed to obtain documentation of a physical examination prior to or within 30 days of child's first day of attendance. Evidence: There was no documentation of a physical in Child B's (date of enrollment 12/8/2015) file. Plan of Correction: Provider will request the information from the parent.
Standard #: 22VAC40-111-70-A Description: Based on review of two children's records, the provider failed to have documentation that parents received all required information in writing before the child's first day of attendance. Evidence: Child A did not have documentation of liability insurance declaration signed by the parent. Plan of Correction: Provider will have parents sign the document and will put in the file.
Standard #: 22VAC40-111-90-A Description: Based on review of two children's records, the provider failed to obtain documentation of the child's immunization record prior to the child's first day of attendance. Evidence: there was no documentation of Child B's (date of enrollment 12/8/2015) immunization record in the file. Plan of Correction: Provider will request the information from the parents.
