54.1-3408 Provider must be MAT certified to administer prescription medication.

An announced initial study was conducted on 2/25/16 between 11:00 and 1:30. The initial inspection had been attempted earlier on 12/16/15, but the provider was not prepared at that time. The provider has removed furniture from the child care room and obtained equipment and furniture for the children. Cots have been obtained and toys are in place. Most required background checks have been received. One required background check is still missing and will be required prior to issuance. There are no children in care. Once licensed, the following information is required to be posted for parents: the license, the licensing inspection forms, the menu and the emergency evacuation plan. A daily attendance sheet listing the names of children as they arrive at the home must be in place. The names will be marked out when the child leaves.

Description: The provider has not obtained all required supplies for the home's first aid kit. Evidence: An inspection of the home's first aid kit found that there were no triangular bandages (at least two are required) and a thermometer could not be located.

Plan of Correction: I will make two triangular bandages and will purchase a thermometer today.

Standard #: 22VAC40-111-800-A
Description: There is no written emergency preparedness plan for the family day home that addresses the most likely to occur emergency scenarios. Evidence: When asked for the home's written emergency plan, the provider stated that she had not completed a plan.

Plan of Correction: Will be going to the library to print off paper work to fill out and will bring to the office no later than 2/26/16.
