Technical Assistance:

An announced initial inspection was made on 4/8/16 at 10:00 a. m. and concluded at 1:00 p. m. The inspector met with the provider and Family Day Home standards were reviewed. There were three family day home children present with points equaling 8 and the provider was caring for the children. The home is set-up with a variety of interest center with many materials and supplies for the children in care. There is no children on any medications at this time, the provider has completed MAT (medication administration trained). One child record was reviewed. Four household member records were reviewed. One observation conducted. Conversations were held with the provider and other household members and three children. The provider had completed an evacuation drill and shelter-in-place drill on 4/6/16.

Description: Based on discussion, the provider could not locate the written emergency preparedness and response plan. Evidence: On 4/8/16, the provider could not locate the written emergency preparedness and response plan that includes: emergency evacuation, emergency relocation and shelter-in-place procedures and most likely to occur scenarios specific to the home, etc.

Plan of Correction: I will locate the emergency preparedness plan and submit to DSS.
