Comments:
Conducted an unannounced monitoring inspection 1:25-2:25pm. Observed 10 children = 23 points with the Provider, Assistant #1, and Assistant #2. Ratios and supervision were in compliance. Most children were napping during the inspection. The one child that was awake was having quiet play time with Assistant #2. According to the menu, the afternoon snack will be: chocolate cookies + apple juice. The daycare areas of the home were found to be very clean, safeguarded appropriately, and well supplied with developmentally appropriate toys and equipment. No medications reported to be on-site. All caregivers are currently certified in First Aid/CPR. Required documents were posted. Record keeping violations found. Description: Based on records reviewed, it was determined that the emergency plan had not been reviewed annually. Evidence: The last documented review was dated July 2014.

Plan of Correction: Emergency Plan was viewed in the same day

Standard #: 22VAC40-111-50-C
Description: Based on observations made, it was determined that identifying information about a child was not kept in a confidential manner. Evidence: The Supplemental Information page of the 7/23/15 inspection report was posted on the bulletin board just inside the daycare entrance. This document contained the name of one child, as associated with the report of the same date. A statement on the documents reads: "Information found on the Supplemental Information page is confidential and this document is not to be posted in the facility."

Plan of Correction: I removed the paper in the same day

Standard #: 22VAC40-191-40-D-1-c
Description: Based on records reviewed, it was determined that caregivers and an adult living in the home did not have a Criminal Record Check (CRC) completed in the last 3 years. Evidence: The most recent CRC's on-file for the Provider, Assistant #1, and Adult #1 were dated March 2013.

Plan of Correction: The Forms was completed and sent [sic]
