54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring inspection was conducted today. Observed 12 children (35 points) directly supervised by 2 caregivers. Ratio was not in compliance as the point system was exceeded by 3 points. A sample of 2 children's records, and 2 caregiver records were reviewed. The physical plant, emergency supplies and procedures were also reviewed. No medications were reported to be on site. The children were engaged in various activities, to include: free play, infant care, hand washing, and rest time. Areas of non-compliance are identified in this report. Description: Based upon documentation reviewed, the provider did not review the emergency plan at least annually and update the plan as needed. Evidence: The emergency plan was last reviewed/updated on 07/14/2014.

Description: Based upon interview, the provider did not obtain from each caregiver at the time of hire and prior to coming into contact with the children a current Report of Tuberculosis (TB) Screening form. Evidence: Caregiver #1 did not have a TB screening on file. The provider confirmed that Caregiver #1 did not have a current TB test. Caregiver #1 was present during today's inspection and has been in contact with the children since her hire date of 03/29/2016.

Description: Based upon observations made, interview, and documentation reviewed, the provider did not ensure that each caregiver did not exceed 16 points. Evidence: 1) There were 12 children present under the supervision of 2 caregivers with a total combined points of 35 points, exceeding the ratio requirement by 3 points. 2) The attendance sheet confirmed that they were 12 children present: 4 infants, 3 toddlers, and 5 preschool children. 3) The provider confirmed that she was aware that she exceeded the point system by 3 points.
