Violations:
Standard #: 22VAC40-111-30-A
Description: Based on the visual inspection of the family day home, the provider did not comply with the terms stipulated on the current conditional license issued. Evidence: Upon arrival to the family day home to conduct an unannounced monitoring inspection, this inspector observed three canines at the storm door with a child. Upon further investigation, the child at the door was enrolled receiving day care services. Based on the stipulations written on the license "Children will not have access to the canines in care," the provider was not in compliance with the terms of the current license issued.

Plan of Correction: The provider corrected the issue on site and informed children in care will no longer have access to canines as it was a misunderstanding.

Standard #: 22VAC40-111-60-B
Description: Based on the inspection of children records, a required component was not listed. Evidence: During the inspection of one child's record, it was observed that the medical policy number was not listed.

Plan of Correction: Per provider: I will obtain the information from the parent today at pickup time.
