Thank you for your time and assistance with the unannounced inspection conducted today, May 3, 2016. The on-site visit was from 11:05 a.m. to 12:30 p.m. On this date, at the time of arrival, there were ten (10) children in the care of the provider and assistant for a total of twenty-four (24) points. There were a total of eleven (11) children enrolled in the program. There were five (5) children's records reviewed and the records for the provider, assistant and one household member were reviewed. Risk scores were discussed. Areas of non-compliance may be found on the violation notice. Areas observed were free choice play, hand washing/bathroom/diapering procedures, lunch, and nap. Lunch was observed to meet the USDA guidelines and consisted of spaghetti with meat sauce, corn, pineapple and milk. Interactions between the provider and children were positive and the children seem comfortable with the daily routine and schedule. This family day home is well equipped with a variety of age/stage materials for the children in care. Children's artwork is displayed throughout the classroom. Standard #: 22VAC40-111-180-A Description: Staff member B did not have documentation of obtaining an updated tuberculosis screening. Evidence: Based on record review and conversation with the staff it was confirmed Staff B had not obtained an updated tuberculosis screening. Last documented update was 2/24/2014. Plan of Correction: Staff B will obtain a tuberculosis screening. Documentation will be sent to the licensing inspector. In the future, updates will be completed every two years.
