Technical Assistance:
Heating of infant bottles was discussed with the provider.

Comments:
A mandated, unannounced monitoring inspection was conducted. The inspector arrived at 11:30 AM and departed at 1:45 PM. Ten children, totaling 24 points were in care with two caregivers. Learning activities and preparation for lunch were observed. A sampling of five children's records were reviewed. Annual training, water safety certification and first aid/CPR training of caregivers were reviewed. It was reported that no medication is administered to children in care and no children's injuries were reported. An exit meeting was conducted with the provider, during which the provider's plans of correction were obtained. Please contact the inspector if there are any questions.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based upon review of five children's records, the provider has not ensured that documentation of a physical examination by or under the direction of a physician is obtained prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. No physical examination is available in the record of child 4 who, according to the record, began attending on 10/28/2015. 2. No physical examination is available in the record of child 5 who, according to the record, began attending on 10/12/2015. 3. The provider was not able to produce physical examinations for either child 4 or child 5.

Plan of Correction: The parent of child 5 was contacted to obtain the required documentation during the inspection. Child 4 is not in attendance today. The parent will be contacted to obtain the required documentation.

Standard #: 22VAC40-111-50-C
Description: Based upon observation of posted information, the provider has not ensured that information regarding the children is kept confidential. Evidence: The Supplemental Information sheet from the previous inspection was posted with the inspection notices on the parent information board. The Supplemental Information sheet states that the information contained is confidential and that the sheet is not to be posted.

Plan of Correction: The Supplemental Information sheet was removed from the board during the inspection and will not be posted in the future.
