Discussion was held on the topics of current enrollment, current staffing, shelter-in-place, and the use of a new swing for infants that prohibits its use over a hard surface. It was suggested to be used over a large cushioned mat. A follow-up was conducted to the inspection conducted in December 2015. All previous violations were corrected except one (please see today's notice for more information).
The following inspection took place between the hours of 1 pm through 4:45 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Ashburn, Virginia. During this unannounced monitoring inspection there were a total of 7 children (15 points) with 2 caregivers and 1 volunteer. A total of 7 children?s files and 4 staff/household member?s files were reviewed. The children were engaged in the following: nap time, snack time, diaper changing, hand washing, music, song, toy play, games, learning, and other organized activities. Standard #: 22VAC40-111-960-E Description: Based on observation, bottles that were refrigerated and labeled with the child's full name was not also labeled with today's date. Evidence - 1. On the date of inspection (05/05/2016), bottles in the child care refrigerator were observed without today's date. 2. This same standard was cited during the previous inspection conducted on 12/01/2015. Plan of Correction: All bottles shall contain both the full name and the current date.
Standard #: 22VAC40-111-100-A Description: Based on review, 1 of 7 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (05/05/2016), the physical for Child #6 was not available for review. 2. The start date for Child #6 was listed as 12/29/2015. Plan of Correction: The Parent of Child #6 shall be contacted and a physical obtained.
Standard #: 22VAC40-111-90-A Description: Based on review, 1 of 7 children's files reviewed indicated that before a child may attend the Family Day Home (FDH), the Provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Evidence - 1. On the date of inspection (05/05/2016), an immunization page for Child #4 was not available for review. 2. The start date for Child#4 was listed as 05/02/2016. Plan of Correction: The Parent of Child #4 will be contacted and an immunization page obtained.
Standard #: 22VAC40-111-170-A Description: Based on review, 1 of 4 staff/household member's files reviewed indicated that the Provider did not obtain from each caregiver at the time of hire prior to coming into contact with children a current Report of Tuberculosis Screening (TB) form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form. Evidence - 1. On the date of inspection (05/05/2016), TB documentation was not available for Member #2. 2. The start date for Member #2 was listed as 02/29/2016. Plan of Correction: TB documentation shall be obtained and placed on file for Member #2.
