Today an unannounced monitoring visit was conducted with Provider. This visit took place in the morning between 9:00-10:55. There were 3 children present with the Provider and 1 assistant for a total of 10 points within the supervision guidelines. The children were observed having morning breakfast, playing with blocks, riding bikes, taking morning nap, and in transition for lunch. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-110-A Description: Based on observation of the children records, one record does not have physical and immunization on the Virginia health form. Evidence: Child #2's record, does not have the results of a physical exam and immunizations recorded on the Virginia Health form. Plan of Correction: Provider stated that she notified parent and is waiting for them to provide it.
