The home has a variety of toys and equipment for children, and child care is provided only on the first floor of the home. The provider was caring for two children with points equaling four. The children were observed having free play with age and stage appropriate toys. One observation was conducted. Conversations were held with the provider and two children. No medications are being administered. The last evacuation drill was documented on 4/23/16. Two children records were reviewed. Household member records were reviewed.

Standard #: 22VAC40-111-70-A Description: Based on review of some children records the provider did not have all required information on file. Evidence: On 5/6/16, after review of two children records, child #2 record did not include page #2 of the Information for Parent's Agreement form signed by parent.
