Technical Assistance:
**Discussed with the provider that if her current household "guest" extends their stay in the home, that they will need to complete a Sworn Disclosure statement, a criminal history record check, and a central registry check as well. Please notify me as soon as possible once this needs to be done.

Comments:
An unannounced monitoring/renewal inspection was conducted today from 8:10am-9:06am. There were 6 children (15 points) directly supervised by 2 staff. The home, 2 staff records, 1 child?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and arriving to care. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-111-800-B
Description: Based on review, the provider failed to ensure that the emergency preparedness plan was annually reviewed and updated as necessary. Evidence: Reviewed the emergency preparedness plan and found that the last update/review was dated 3/11/15.

Plan of Correction: Updated plan

Standard #: 22VAC40-111-120-B
Description: Based on review, the provider failed to ensure that 2 reference checks were completed prior to the assistant's first day of employment. Evidence: Reviewed 2 staff records and found that Staff 2 did not have documentation of reference checks.

Plan of Correction: Obtained references

Standard #: 22VAC40-111-60-B
Description: Based on review, the provider failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 1 child's record and found that the file was missing... 1) documentation of medical insurance and the medical insurance policy number 2) documentation of parent's being informed of the provider's liability insurance status.

Plan of Correction: Obtain policy number and obtained signed liability insurance form.
