Comments:
An announced Initial inspection was conducted today. There are 4 children present (9 points) with one adult. 1 adult and 2 children's files were reviewed. See violation notice for missing information. All children's files would be missing the same information. Children were observed playing, using the bathroom and washing hands. The crib assembled for use has a manufacture date of January 2016. If you have any questions regarding this inspection, please contact me at 703-479-4678. LI was present today from 10:05 AM - 11:10 AM. Due to case load adjustments, your next inspection may be with a different inspector. All inspectors carry ID badges for security purposes.

Standard #: 22VAC40-111-60-B
Description: Based upon a review of files, children's files re incomplete. Evidence: 2 of 2 files reviewed were missing documentation of 2 complete emergency contacts addresses. (Children 1,2)

Plan of Correction: I will use the DSS model form to have all required information on file.

Standard #: 22VAC40-111-70-A
Description: Based upon a review of files, children's files are missing documentation of receiving written policies and procedures. Evidence: 2 of 2 files reviewed were missing written acknowledgment of required policies and procedures. Missing information includes but is not limited to: DSS web site address, Custodial rights statement, notification of pets in the home.

Plan of Correction: I will use the model DSS form. I will complete the form and have the parents sign the document.
