An unannounced, mandated monitoring inspection was conducted. The inspector arrived at 1:25 PM and departed at 2:30 PM. Twelve children plus one household child, a total of 23 points, were in care with two caregivers. A substitute provider was on duty during the inspection. Transition from lunch time to nap time was observed. Three caregiver records, one household member record and four children's records were reviewed. It was reported that no medication is administered to children in care. No children's injuries were reported. An exit meeting was conducted with the substitute provider prior to conclusion of the inspection. Upon receipt of the inspection notices, the provider must develop a plan of correction for each violation cited. The plan of correction must include the following: - The steps taken to correct the noncompliance with the standards (violations); - Measures to prevent reoccurrence of noncompliance; - Person(s) responsible for implementation and monitoring of preventive measures(s); - Date by which noncompliance will be corrected. The provider has ten calendar days from receipt of the inspection notices to complete the sections titled Plan of Correction and Date to be Corrected, sign and date the pages where indicated and return the inspection notices to the Licensing Office. A copy is to be retained to be posted at the family day home. (NOTE: Supplemental Information is not to be posted due to confidentiality). Results of the inspection notices are subject to public disclosure and will be posted on the VDSS web site within 15 calendar days, regardless of whether the Plan of Correction section has been completed. If a provider disagrees with a violation(s), the facility has 15 calendar days from receipt of the inspection notices to initiate a written request for a First Step Review. Standard #: 22VAC40-111-830-A Description: Based upon observation, the facility has not ensured that emergency evacuation procedures are practiced monthly. Evidence: The posted evacuation drill chart does not have documented an evacuation drill conducted in April 2016. Plan of Correction: The provider responded with the following: We actually did do this drill on 4/15/16. After we came in, I had forgotten to Documentate it on our Evacuation Drill form. Wrote in the event and will be sure not to forget again to documentate it.
Standard #: 22VAC40-111-150-D-1 Description: Based upon review of three caregiver records, the provider has not ensured that assistant caregivers have current certification in cardiopulmonary resuscitation (CPR). Evidence: 1. The record for caregiver 3 does not include documentation of current CPR certification. 2. Caregiver 3 was uncertain when her certification expires or the location of the documentation. Plan of Correction: The provider responded with the following: Have scheduled CPR/1st Aid class for 6/30/16 Will be sure that even on call or backup assts. will have their certificates current.
