Comments:
Today an unannounced monitoring visit was conducted with the Provider. This visit took place in the morning between 9:30am-12 there were 6 children with the Provider and 1 assistant for a total of 24 points within the supervision guidelines. The children were observed arriving, having morning story-time, learning to count, having free play and in transition for lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, attendance records, fire drill log, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-B
Description: Based on record review of three children's files, two files did not include documentation of two designated emergency contact persons. Evidence: Child #2's file does not have the father's work address. Child #4's file does not have the two emergency contact person's address in the event of an emergency.

Plan of Correction: Provider stated that she will get the information today from the parents.

Standard #: 22VAC40-111-690-D
Description: Based on observation of the diapering area, the diaper changing surface was found ripped/torn and no longer nonabsorbent. Evidence: The diaper changing pad was found ripped/torn.

Plan of Correction: Provider stated that she will replace it this weekend.
