54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 10:45 am - 11:45 am. There were three children in care with the provider ages 16 months, 21 months, and 2 years for a total of 8 points. The children were observed participating in free play. Lunch served today is tuna sandwich on wheat bread, green beans, watermelon, and milk. Three children's records were reviewed. The provider's record was reviewed and in compliance. The provider is current in CPR and First Aid. Standard #: 22VAC40-111-100-A Description: Based on documentation of three children's records it was determined that the provider did not obtain a physical for all children within the first 30 days of enrollment. Evidence: Child B with a start date of 11/17/2015 did not have documentation of a physical on file. Plan of Correction: The provider will request an up-to-date physical from the parents of child B.
Standard #: 22VAC40-111-60-B Description: Based on observation of three children's records it was determined that not all records were complete. Child A with a start date of 5/9/2016 did not have documentation of verification of identity such as birth certificate, proof of birth letter, or passport. Plan of Correction: The provider will request proof of identity from the parent of Child A.
Standard #: 22VAC40-111-90-A Description: Based on observation of three children's records it was determined that not all records contained documentation of immunizations before the child's first day of enrollment. Evidence Child A with a start date of 5/9/2016 and Child B with a start date of 11/17/2015 did not have documentation of immunizations on file. Plan of Correction: The provider will request a copy of up-to-date immunizations from the parents of child A and child B.
Standard #: 22VAC40-111-320-E Description: Based on observation of the family day home it was determined that not all portable heaters were inaccessible to children. Evidence: The LI (Licensing Inspector) observed a portable electric heater on the floor accessible to children. Plan of Correction: The provider turned off the heater during the inspection and will move the heater to an area inaccessible to children.
