*TB/PPD screenings are accepted by the Division. You do not need to do the TB/PPD test or X-ray unless directed by your doctor. **Your granddaughter can visit and be considered a resident child. She would not count in the overall capacity of 12 for your license. She would, however, count in total number of points.

An unannounced monitoring inspection was conducted with provider and assistant. There were eleven day care children present, ranging in age from 1 years to 5 years old, totalling 27 points. The children had finished lunch and were watching an age appropriate TV show for a few minutes to wind down for rest time. The children were given books to read and laid down for rest. Three children's files were reviewed - please see Supplemental Summary for missing items. Provider's first aid and CPR certification is valid until 2/2017 and Medication Administration Training is valid until 1/25/2018. There are no children receiving medication today. Description: Based on review of three children's records, the provider failed to obtain documentation of a physical prior to the child's attendance or within 30 days after the first day of attendance. Evidence: Child A (date of enrollment 8/24/2015) and Child B (date of enrollment 10/15/2015) did not have documentation of a physical or well baby exam in the file.

Plan of Correction: Provider will request the information from the parents again.

Standard #: 22VAC40-111-530-A
Description: Based on observation, the provider failed to have a top cover for the children during rest time. Evidence: the children each had a bottom cover (sheet) for the cribs and rest mats, but the children were not furnished with a top covering.

Plan of Correction: Provider will ask parents to provide a top covering for the children during rest time.

Standard #: 22VAC40-111-690-E
Description: Based on observation, the provider did not dispose of soiled diapers in a leak proof storage system that is foot operated. Evidence: four diapers changed before rest time were put in a small bread bag on a shelf near the diaper pad to be taken out to the garbage. The diapers were not disposed of in the foot operated diaper pail, in such a way that neither the caregivers hand or the soiled diaper touches the exterior surface during disposal.

Plan of Correction: Providers will dispose of diapers in the foot operated diaper pail and will take the bags out to the trash.
