An unannounced monitoring inspection was done today June 1, 2016 beginning at 8:45 AM and ending at 12:15 PM. There were six children in care today. Three were present with the provider and points were at six. as the inspector arrived. The assistant arrived at 8:45 AM with three more children and points were then at seven. The children were watching an educational show when the inspector arrived. They ate breakfast and then went to the play room for free choice play. Outside time and lunch were also observed. A sample of children's files and staff/household files were reviewed. The provider does not administer medicine and continues to participate with the USDA program. Standard #: 22VAC40-111-170-B Description: Based on observation the provider did not have all the required information for the TB test of an assistant. Evidence: 1. During the inspection the LI reviewed the files of assistant # 1 and the TB test is current however it is not signed by a physican or designee of the physician's office. Plan of Correction: The assistant thought she had had this done however the verification could not be found. The assistant will have this corrected.
Standard #: 22VAC40-111-280 Description: Based on observation the provider did not keep all cleaning products out of reach of the children. Evidence: 1. During the inspection the LI observed an can of air freshner on the tub in the bathroom used by the children. Plan of Correction: This was removed during the inspection and placed in an area inaccessible to the children. This is not normally in this bathroom. Provider will keep a check.
Standard #: 22VAC40-111-320-B Description: Based on observation the provider did not cover all of the electrical outlets that were accessible to children under the age of five. 1. During the inspection the LI observed and electrical outlet not covered with a protective cap in the main play room. This was over a shelf approximately 4' high. Plan of Correction: The outlet was covered during the inspection. The provider and assistant will keep a check on these outlets.
