22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted on 06/03/2016 from 11:00am to 12:15pm. Children were observed playing with play dough, playing with manipulatives, using dramatic play items, diaper changing, and preparing for lunch. Two new children's records were reviewed and required information was updated in staff records. There were no medications at site for child present. There were 7 children present today (19 points) with the Provider and assistant caregiver. An exit interview was conducted on site at the conclusion of the inspection. Standard #: 22VAC40-111-750-A Description: Based on observation, a topical skin product was not labeled with the child's name. Evidence: Bug repellent for a child was not labeled with the child's name. Plan of Correction: Provider will label the repellent with the child's name.
Standard #: 22VAC40-111-100-A Description: Based on record review, there is no documentation of a physical examination under direction of a physician prior to child's first day of attendance. Evidence: Child #6 started on 02/15/2016 and does not have documentation of a physical examination in the record. Plan of Correction: Provider will contact the parent and provide them with the paperwork to complete and provide documentation of a physical examination.
Standard #: 22VAC40-111-540-A Description: Based on observation, two children were placed in high chairs without being securely fastened. Evidence: One child was observed in a high chair without straps. Another child was in a high chair without being securely fastened with the safety straps available. Plan of Correction: Going forward provider will utilize high chairs with straps. Because on of the straps was taken off to wash and clean. She will clean and disinfect high chair pads at the end of the day.
