Standard #: 22VAC40-111-60-B Description: Based on review of children's records the provider did not obtain all required information. Evidence: Child #1 was missing a recorded first day of attendance. Child # 2 was missing a recorded first day of attendance. Child # 3 was missing a recorded first day of attendance. Child #4 was missing complete work addresses and telephone numbers for both parents. Plan of Correction: Provider will document the first date of attendance. The provider will ask the parents to update their work information.
Standard #: 22VAC40-111-320-A Description: Based on observation of the living area of the family day home the provider did not ensure all small electrical appliances were unplugged while not in use. Evidence: A coffee maker was found plugged in in the upstairs kitchen in an area not used by children. Plan of Correction: Provider will unplug the coffee maker and ensure it is unplugged whennot in use.
Standard #: 22VAC40-111-320-B Description: Based on observation not all outlets had protective covers installed. Evidence: A power strip located in the child care area did not have protective covers on three of the outlets. Plan of Correction: The provider will locate and install protective coverings on the outlets.
