54.1-3408 Provider must be MAT certified to administer prescription medication.
An announced initial inspection was conducted to day between 12:00pm and 12:55pm today. There were four children in care (9 points) under the direct supervision of the provider. Provider and children's records were reviewed and an inspection of the living areas of the house was conducted. Children were observed having story time, playing instruments and transitioning to nap time. Violations found during this inspection can be found on the violation report. Standard #: 22VAC40-111-60-B Description: Based on review of children's records the provider did not obtain all required information. Evidence: Child #1 was missing a recorded first day of attendance. Child # 2 was missing a recorded first day of attendance. Child # 3 was missing a recorded first day of attendance. Child #4 was missing complete work addresses and telephone numbers for both parents. Plan of Correction: Provider will document the first date of attendance. The provider will ask the parents to update their work information.
Standard #: 22VAC40-111-320-A Description: Based on observation of the living area of the family day home the provider did not ensure all small electrical appliances were unplugged while not in use. Evidence: A coffee maker was found plugged in in the upstairs kitchen in an area not used by children. Plan of Correction: Provider will unplug the coffee maker and ensure it is unplugged whennot in use.
Standard #: 22VAC40-111-320-B Description: Based on observation not all outlets had protective covers installed. Evidence: A power strip located in the child care area did not have protective covers on three of the outlets. Plan of Correction: The provider will locate and install protective coverings on the outlets.
