Description: Based upon a review of one child's file, the file is incomplete. Evidence: Child 1's file is missing: father's information, complete emergency contact information (address), doctor's information, health insurance information.

Plan of Correction: I have the aprent complete the model DSS forms for the child.

Standard #: 22VAC40-111-70-A
Description: Based upon a review of files, there is no documentation that parents have been provided all required policies and procedures. Evidence: One child's file was missing written acknowledgement that parents are provided all required policies and procedures. Missing information includes but is not limited to: Custodial right's statement, DSS web site address, check in/out procedures, termination policy.

Plan of Correction: I will complete the model DSS form and have the parent sign the forms.

Standard #: 22VAC40-111-320-G
Description: Based upon a review of documentation, there is no chimney inspection available for review. While the chimney is not used, it is not permanently disabled and must be inspected.

Plan of Correction: An inspection was scheduled during today's visit.
