54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today from approximately 3:00 to 4:30pm. Twelve children were present with the provider and two assistants. Total 35 points with three caregivers. The children were eating snack, playing with toys inside and playing outside this afternoon. One assistant is new since the last inspection. Her record was reviewed today. Four children are new since the last inspection and their records were reviewed today. Remember to write all substitutions on the menu. If you have any questions, I can be reached at 703-479-4709.
Standard #: 22VAC40-111-120-B Description: Based on record review, it was found that there was no documentation of reference checks for the new assistant who started on May 23,2016. Plan of Correction: Reference checks for the assistant will be obtained and documented in her record.
Standard #: 22VAC40-111-90-A Description: Based on record review, no documentation of immunizations was found for one child who started in care on 6/13/2016. Plan of Correction: The immunization record will be obtained from the parent.
