Provided the provider with copies of the following: sunscreen form, general field trip form, fire drill log and assistant provider record form, orientation and references.
The following inspection took place between the hours of 10:15 am through 11:45. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced renewal inspection there were a total of 6 children, including 1 of the providers own children (12 points) with 2 caregivers. A total of 3 children's files and 5 staff/household member's files were reviewed. The children were engaged in the following: playing outside at the neighborhood playground, legos, tracing numbers, phonics and hand washing. The violations were reviewed with the provider. Standard #: 22VAC40-111-750-A Description: Based on record review and staff interview, the provider has not maintained written authorization to apply sunscreen. Evidence: The assistant stated that they apply sunscreen to children. There was no written authorization from the parents to allow the provider to apply the sunscreen. Plan of Correction: I will have the parents complete the authorization to apply non prescription topical skin product form and file in each child's record.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records and observation, children records do not have written authorization for the child to be taken off the premises of the family day home. Evidence: Upon the licensing inspector's arrival, the children and assistant were observed at the playground across the street from the family day home. The children's records reviewed did not have written authorization (general permission for regularly scheduled trips) for being taken off the premises of the family day home. Plan of Correction: I will have the parents complete the regularly scheduled field trip form and place in each child's record.
