Technical Assistance:
Discussed with provider: I am not able to submit the application for approval until Adult C has moved from the home. Please contact me once that move has occurred, to inspect the home again.

Comments:
An announced Initial study was conducted today. There are 4 children (13 points) with 2 adult caregivers. 4 adult and 4 children's files were reviewed. See violation notice for missing information. Children were observed playing during today's inspection. There are 4 cribs assembled for use with acceptable manufacture dates. I recommend sheet tightening strips for the smaller mattress. Due to case load adjustments, your next inspection may be with a different inspector. LI was present today from 9:50 AM - 11:20 AM. Standard #: 22VAC40-111-60-B
Description: Based upon a review of files, one child's file is incomplete. Evidence: Child 1's file is missing: first day of attendance, Health insurance policy number, second emergency contact address

Plan of Correction: I will ask the parent to complete the missing information.

Standard #: 22VAC40-111-70-A
Description: Based upon a review of files, parents have not been provided all required policies prior to the first day of attendance. Evidence: 4 of 4 files reviewed were missing the decision to administer medication. (Children 1-4)

Plan of Correction: I will have parents sign the model form today.

Standard #: 22VAC40-191-40-B
Description: Based upon a review of files, one adult file contains a disqualifying background search. Evidence: Adult C's background search contains a completed Criminal Record Check (CRC) with a barrier conviction.

Plan of Correction: The person will move out o the house.
