Comments:
An unannounced renewal inspection was conducted today, July 20, 2016 to determine the provider's compliance with licensing standards. The inspection was initiated at 2:55pm and concluded approximately 4:15pm. In attendance were 5 children totaling 18 points being supervised by the provider and assistant. Children were interacting with staff, playing with toys, and coloring pictures. Children books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. All rooms of the home were inspected with no hazards found and smoke detectors present. The family day home had the required postings displayed on the bulletin board upon entering the main children's room. The first aid kit, fire extinguisher, and emergency supplies were inspected. Three children and two provider records were inspected with the majority of the required information found. The following was also inspected: fire drill log, shelter-in-place drills, emergency preparedness plan, parent agreements, and other pertinent information. Standard #: 22VAC40-111-80-A
Description: Based on the inspection of children records, the required information regarding a child's identity was not present. Evidence: During the inspection of three children records, child #1 did not have verification of the child's identity in the record. Licensing standards require this information within seven days of the child's first day of attendance.

Plan of Correction: Per provider: I'll get it from the mother by tomorrow and fill out the back of page 2.

Standard #: 22VAC40-111-200-A
Description: Based on the inspection of provider records, the required information was not present. Evidence: During the inspection of two provider records, it was discovered that staff #1 did not have documentation of orientation. Orientation shall be conducted by the end of the the first week of assuming job responsibilities.

Plan of Correction: Per provider: I will go over the information on the form with the provider and we will both sign it.
