Technical Assistance:
Sixteen training hours for assistant #2 should be completed by the end of August to meet the training requirement.

Comments:
An unannounced monitoring visit was conducted today. Nine children were present with two caregivers for twenty-four points which is within ratios for supervision. Four children were napping upon arrival with the remaining five playing with materials and interacting with the caregivers. Children were finishing lunch and had played outside in the morning upon arrival. Lunch served today was quinoa with chicken, cantaloupe and milk. No medication was reported to be on-site. Reviewed wading permission forms as well as ensuring that only children who are potty trained can use wading pool and two caregivers must be present at all times. Areas of non-compliance are identified in this report. Please sign and date the report and return it to me within 10 calendar days from date of receipt. Description: Based on the evacuation log, a monthly fire drill was not conducted one month. Evidence: The provider and children did not practice a drill in April 2016. The provider had a shelter-in place drill that month.

Description: Based on record review, 2 of 3 children's records were incomplete. Evidence: Child #1 was missing allergy and chronic problems documentation as well as a parent work address. Child #2 was missing an emergency contact address.

Description: Based on record review, 1 of 3 written acknowledgement of the Parent Information forms is missing a parent signature.
