54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted on Monday, July 25, 2016. Present today were three children totaling eight points with the provider. An infant was observed swinging in an infant swing, smiling and having a bottle. The older two children were playing with trucks and coloring. PM Snack: vanilla wafers and bananas Practice evacuation drill: 7/13/16 The provider is currently certified in first aid and CPR. No transportation is provided. One child's record and three household records were inspected.
Standard #: 22VAC40-111-180-A Description: Based on review of household records, the provider did not ensure all tuberculosis (TB) reports were up to date. Evidence: Household #2 TB expired 3/5/16. Plan of Correction: Per Provider: " Will send #2 for TB."
