54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 9:30 am - 10:45 am. There were 9 children in care with the provider and one assistant ages, 4 months, 9 months, 2 years, 2 years, 2 years, 3 years, 3 years, 3 years, and 4 years for a total of 22 points. Lunch served today is cheese pizza with tomato sauce, cantaloupe, cheese, and milk. The children were observed participating in an craft project. Nine children's records were reviewed and in compliance. The provider and assistant record were reviewed and in compliance. The most recent emergency escape drill was conducted 7/6/2016. The provider is current in CPR, First Aid, and MAT(Medication Administration Training). Standard #: 22VAC40-111-240-A Description: Based on observation of the family day home it was determined that not all areas of the home were maintained in safe and operable condition. Evidence: 1. In the play room of the family day home there are five holes in the wall with exposed sheet rock and chipping paint. 2. In the play room of the family day home there is speaker wire hanging out of an outlet face plate exposed to the children. 3. Crib #3 in the nursery is missing one wheel. 4. There is peeling a chipped paint on the wall of the nursery. Plan of Correction: The provider will cover the holes until the holes can be repaired and the walls can be painted. The speaker wire will be removed and placed inside the face plate to be inaccessible. The provider will replace the missing wheel of the crib.
