Comments:
An unannounced monitoring inspection was completed on 7/27/2016. There were seven children present with the provider and assistant supervising. The point total was 12. Four children's records were reviewed. The provider, household, and assistant's records were all reveiwed. The inspector arrived at the provider's home at 11:30 am and departed at 1:45 pm. The children were observed during outside time, during free play time, during lunch, and during nap. There was discussion with the provider about the following: updates needed for two parts of the background check for the provider and household member and about location of electric fan in the school age room. Child # 3 is on a conditional enrollment for immunizations and will need to be update 8/2016.

Standard #: 22VAC40-111-60-B
Description: Based on review of four children's records, the licensee failed to ensure that all records were complete. Evidence: 1. Child # 1 was missing an complete address for one of the two required emergency contacts and start date.

Plan of Correction: The licensee will obtain the missing information and will monitor all records.
