54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring/renewal inspection was conducted today at 9:55am. There were 10 children (25 points) directly supervised by 1 staff. A second staff person arrived approximately 15-20 minutes after the inspector's arrival. The home, 2 staff records, 1 child?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely outside and preparing for snack. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-111-110-B Description: Based on review, the provider failed to ensure that immunization records are signed by a licensed physician, the physician's designee or an official of a local health department. Evidence: Reviewed 1 child's record and found that they were missing documentation of a signed immunization record. Plan of Correction: Will take care of that today and have them sign.
Standard #: 22VAC40-111-60-B Description: Based on review, the provider failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 1 child's record and found the following... 1) There was no documentation of 2 emergency contacts 2) There was no documentation of the medical insurance policy number. 3) There was no signature verifying that the parents received documentation on the medication policy and emergency plan. Plan of Correction: Will notify parent today.
Standard #: 22VAC40-111-240-A Description: Based on observation, the provider failed to ensure that the areas and furnishings of the FDH, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1) Observed a large crack and hole on the base of the slide near the rear of the outside play area. 2) Observed cracked seats along a circular seating "ride". Plan of Correction: I will remove the slide and fix the seats.
Standard #: 22VAC40-111-570-A Description: Based on observation, the provider failed to ensure that caregivers do not exceed 16 points by using the provided point system to determine if an additional caregiver is needed. Evidence: Observed that upon arrival there was 1 caregiver, 10 children (25pts), and 2 of the provider's own children. The provider arrived approximately 15-20 minutes after the licensing inspector's arrival. Plan of Correction: Will have to plan better next time.
