Technical Assistance:

An unannounced monitoring inspection was made on 8/3/16 at 12:30 and concluded at 2:00 p. m. The inspector met with the provider and some but not all Family Day Home standards were reviewed. There were five family day home children plus one grandchild present with points equaling 11, the provider was caring for the children. The children were observed having lunch which consisted of Pizza, Green Beans, Tropical Fruit Mix and Milk. After lunch the children were observed preparing for nap time. Five children records were reviewed. Conversations were held with the provider and all children. Observations were made in the day care room. No children on medications today. No new household members or caregivers. The last monthly evacuation drill was conducted on 7/15/16. The last shelter-in-place drill was conducted on 7/15/16.

Description: Based on review of children records the family day home did not have all required information on file. Evidence: On 8/3/16, child #1 record was missing emergency contact addresses, emergency preparedness plan, policy for medications, liability insurance disclosure, and parent agreement-information for parents form. Child #2 record was missing name of insurance and policy number.

Plan of Correction: I will request the forms from the parents.
