An announced Initial inspection was conducted today. There are 3 children present (9 points) with one provider. Children were observed resting, washing hands, using the bathroom and finishing lunch during today's inspection. 3 adult and 3 children's files were reviewed. See violation notice form missing items. There are 3 cribs assembled for use.(2013, 2014) Due to case load adjustments, your next inspection may be with a different inspector. All inspectors a carry identification for security purposes. Description: Based upon a review of files, children's files are incomplete. Evidence: Missing information includes but may not be limited to: Second Emergency contact, Health Insurance number (Child 1) Emergency Authorization (Child 2,3)

Plan of Correction: I will ask the parents to complete the forms.

Description: Based upon a review of files, parents have not acknowledged all required policies and procedures. Evidence: 3 of 3 files reviewed were missing signatures, acknowledging required policies and procedures. Missing policies include but are not limited to : Check in and out, termination policy. (Children 1,2,3)

Plan of Correction: I will have the parents sign the forms.

Description: There is no step on trash can available for diaper changes.

Plan of Correction: I will purchase 2 trash cans for diapering.
