22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted on August 11, 2016 from approximately 10:00 am to 11:30 am. Provider and assistant were present with three children. Children were observed during free play. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, emergency evacuation drill documentation, children's records, caregiver & household member records, emergency supplies and required postings. Violations were reviewed with and verified by the provider during the exit interview conducted on this date.
Standard #: 22VAC40-111-800-B Description: Based on review, facility did not ensure the emergency preparedness plan is reviewed at least annually and document in writing the review. Evidence: On August 11, 2016, documentation stated the emergency preparedness plan was last reviewed on February 20, 2013. Plan of Correction: Provider will review it and sign it.
Standard #: 22VAC40-111-180-A Description: Based on review, provider did not ensure each caregiver and adult household member has an updated Tuberculosis test/screening every 2 years. Evidence: On August 11, 2016, the following was found during record review: 1. The record for caregiver #1 has a most recent TB test dated April 21, 2014. 2. The record for household member #1 has a most recent TB test dated April 21, 2014. Plan of Correction: They will have the screening completed tomorrow.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review, provider did not ensure household member background checks are updated every 3 years. Evidence: On August 11, 2016, the following was found: 1. The record for household member #1 has a most recent criminal history check dated March 13, 2013. 2. The record for caretaker #1 has a most recent criminal history check dated April 29, 2013. Plan of Correction: They will go to the bank tomorrow and get it notarized to place in the mail.
