Reviewed with provider possible choking hazards.

An unannounced inspection for license renewal was conducted today from approximately 11:30am to 1:30pm. Seven children were present with the provider and assistant. Total 19 points with two caregivers. The children were observed playing with table toys, floor toys, eating lunch and transitioning to rest time. Four children are new since the last inspection. Their records were reviewed today. The provider has not yet renewed her Medication Administration Training, and so she is not administering any medication until she obtains her updated certification. Standard #: 22VAC40-111-750-B
Description: Based on observation, it was found that the tubes of diaper ointment located in the drawers under the changing table were accessible to the children.

Plan of Correction: The diaper ointment was removed and moved to higher location that is not accessible to the children.

Standard #: 22VAC40-111-900-B
Description: Based on observation, it was found that a child was drinking from a bottle while lying down in the crib.

Plan of Correction: The child was removed from the crib and was seated until he finished drinking the bottle. Children will not be given anything to drink while lying down.

Standard #: 22VAC40-111-960-E
Description: Based on observation, it was found that one bottle in the refrigerator was not labeled with the child's name and date.

Plan of Correction: The bottle was labeled at the time of the inspection.

Standard #: 22VAC40-111-100-A
Description: Based on record review, it was found that there was no documentation of a physical exam for Child #1, who has been in care for over 30 days.

Plan of Correction: Documentation of a physical exam for this child will be obtained.
